Physical Cards

Each player must have a current physical (cleared to play sports, signed/dated on or after April 1, 2008) on file with our Club before the player will be allowed to participate in practice or game play.

There will be no exceptions.

Please make arrangements to have physicals completed prior to the first day of practice. We accept copies of the WIAA physical form (click here for form) that is required by the Oshkosh school district. Your doctor's office should have these cards available to be filled out when your child has their physical.

If your child has a current physical on file at school, make sure you get a copy of it before the end of the current school year, and make sure that it will still be effective (dated on or after April 1, 2008) for the upcoming 2009-2010 school year.

We start practice in early August, well before the school staff has returned to the offices. If you wait until the start of practice to try to get the physical, your child may miss several days and even weeks of practice. The schools have informed us that we are unable to access copies of the players' physicals - they MUST be obtained by a parent.
